CREATING TRAINING PROGRAM TRAINER INDIVIDUAL Information to collect from students: First Name

Responsibilities of Registered Training Organisation Collect a USI from each student Verify a USI supplied by a student Ensure a student has a valid USI before issuing a statement of attainment Ensure the security of USIs and related documentation Destroy any personal information which you collected solely for the purpose of applying for a USI on behalf of a student Adhere to all legislative requirements under the USI legislative

USI Creation - responsibility of student as part of enrolment process Information needed for each form of ID http://www.usi.gov.au/Training-Organisations/Pages/proof-of-id.aspx REQUIRED DOCUMENTS -

Every student studying has to fill in and submit the enrolment form together (students studying in Australia have to hold a USI number) - Submit at least 10% of all students assessment tools (e.g. Knowledge Assessment Answer Sheets, Incident Reports etc.) and retain copies of all other student documents for a minimum period of six (6) months after course completion

- End of course survey (AQTF Questionnaire) to be submitted by every student - Submit one RTO Student Assessment Record Sheet by every student
